1. 뇌성마비(CP)는 태아기 또는 유아기 뇌의 병변으로 기인된 비진행성 뇌발달의 손상으로 인하여 발생된 영구적인 복합성 근육장애이다1). 소아에서 만성적인 장애를 유발하는 원인 중 가장 흔한 것의 하나로2), 발생률은 1000명의 신생아 출생 당 2명 정도이다3).

 뇌의 손상은 운동기능의 장애를 일으켜 마비palsy, 약화weakness, 불협incordination 등을 나타내며 운동기능의 기동성mobility과 조작기능dexterity의 장애를 나타내게 된다4). 움직임과 자세발달에 영향을 미치고 활동의 제한을 가져오는 운동장애는 뇌성마비의 주요장애요인으로 나타난다. 대부분의 뇌성마비 아동은 경직성(spasticity), 근 수축(muscle contractures), 협응장애(dyscoordination), 선택적 운동조절 소실, 근력약화(muscle weakness)를 포함하는 많은 신경학적 손상을 가지며, 이로 인해 운동기능과 일상 활동의 제약을 받는다5). 이러한 발달의 정체는 신체의 안정성과 운동성을 떨어뜨려 비정상적인 근육의 비정상적인 발달이 나타나게 된다6).

 고관절 아탈구 및 탈구는 뇌성마비 환아들에서 발생할 수 있는 합병증들 중 하나로 그 발생 빈도가 높은 것으로 알려져 있으며 적절한 치료를 하지 않을 경우에는 앉는 자세와 보행이상, 척추 측만증, 고관절 동통, 회음부 위생의 지장 등을 초래하게 된다7). 뇌성마비 환아의 고관절 아탈구는 대개 2세에서 10세 사이에 발생한다고 알려져 있다8). 고관절 이형성은 결국 신체의 기능장애와 관련되므로9), 골격계가 성숙한 뒤 치료를 하면 치료의 결과는 효과적이지 못하다. 고관절이 관절 가동범위를 유지하고 통증없이 안전하게 성장하기 위해서는 5세 전후에 관골구내에 대퇴골두가 중앙화(centralization) 되는 것이 필요하다고 하였다10). 이러한 문제를 해결하기 위한 다양한 중재방법으로 의료적 치료와 재활이 있으며 이를 보조해 줄 수 있는 교정제품이 필요하다. 특히, 중증 뇌성마비 환아의 경우, 고관절 탈구가 진행되면 신체적, 비용적, 심리적인 문제가 매우 크게 발생하나 이를 보호‧예방해 줄 수 있는 교정 제품에 대한 연구는 매우 부족한 실정이다.

 본 연구에서는 중증 뇌성마비 환아의 고관절 탈구 예방을 위해서, 고관절 아탈구가 발생하기 이전부터 일상적 착용을 가능케 해 고관절 탈구‧아탈구를 방지하여 통증 및 외관의 변형을 감소시키고 뇌성마비 환아의 정상적인 발달에 도움을 줄 수 있는 착용형 교정제품 패턴을 제시하고자 한다.

1. Cerebral palsy (CP) is a permanent, complex, muscular disorder caused by damage to non-progressive brain development due to brain lesions in infants during the prenatal period1). It is one of the most common causes of chronic disabilities in children2), and its incidence rate is approximately 2 per 1,000 births3).

Brain damage causes impairment of motor functions, resulting in palsy, weakness, and incoordination. Patients who suffer from brain damage display mobility and motor function dexterity impairments4). Movement disorders affect movement and postural development, and lead to activity limitations. They present a major obstacle to CP patients. Most children with CP have a number of neurological impairments, including spasticity, muscle contractures, discoordination, loss of selective motor control, and muscular weakness, resulting in limitations in motor functions and daily activities5). Such a stagnation of development reduces the stability and mobility of the body, causing abnormal development of abnormal muscles6).

Hip subluxation and dislocation are known to be expressed symptomatically in high incidence among complications that can occur in children with cerebral palsy. If not properly treated, then they can lead to problems with seated posture and gait, scoliosis, hip pain, and impairment of perineal hygiene7). Hip subluxation in children with CP is known to generally occur between ages 2 and 108). Since hip dysplasia is ultimately associated with bodily dysfunction9), treatment is ineffective if performed after the skeletal system’s maturation. It is necessary to centralize the femoral head in the zygomatic cavity before and after age 5, to enable the hip joint to maintain appropriate range of motion and to develop safely without pain10). There are various interventions to rectify these issues, including medical treatment and rehabilitation, and an orthotic device is necessary to help in those interventions. In particular, in the case of children with severe CP, when hip dislocation occurs, severe physical, psychological, and financial problems also arise. However, research on orthotic devices that protect the patient and prevent the aforementioned issues is scarce.

This study suggests a wearable orthotic device pattern that can reduce pain and deformation, and one that could help in the normal development of children with cerebral palsy. Such a pattern enables daily wear prior to the occurrence of hip subluxation to prevent hip dislocation and subluxation.
