혈액투석환자의 자가간호수행은 만성적인 질병을 효과적으로 관리하여 삶의 질을 높이는 중요한 요인으로 여겨져 왔다.

대상자의 자가간호수행은 투석 중 흔하게 나타나는 신체적 요인인 수면장애와 심리적 요인인 우울, 투석으로 인한 스트레스 상황을 이겨내는 극복력과 연관이 있는 것으로 확인되었다. 따라서 본 연구는 혈액투석환자의 자가간호수행에영향을 미치는 요인을 규명함으로써 자가간호수행을 향상시키기 위한 간호 중재 프로그램 개발을 위한 기초자료를제공하고자 한다.

본 연구 설계는 서술적 조사연구이며, 연구대상은 B 광역시 일개 상급종합병원 인공신장실에서 혈액투석을 시행받는 84명을 대상으로 하였다. 자료수집기간은 2020년 8 월 17일부터 동년 9 월 5 일까지였으며, 자료수집 방법은 K 대학교의 기관생명윤리위원회의 승인을 받은 후 B 광역시 소재의 K 대학병원 간호부에 허가를 받아, 구조화된 자가 보고형 설문지를 사용하여 수집하였다. 연구도구로 자가간호수행 측정 도구는 송미령(1999)이 혈액투석환자를 대상으로 개발하여 조미경과 최명애(2007)가 수정·보완한 도구를, 수면장애는 오진주, 송미순, 김신미(1998)가 개발한 주관적 수면측정도구를 연구자가 수정, 보완하여 사용하였다. 우울은 Beck(1967)의 우울척도 (Beck Depression Inventory)를 한홍무 등(1986)이 한국의 정상 집단을 중심으로 표준화시킨 도구(BDI-Ⅰ)를 사용하였고 극복력은 김은영(2019)이 혈액투석환자를 대상으로 개발한 자가 보고형 극복력 측정도구를 사용하였다. 자료분석 방법은 IBM SPSS WIN 25.0 program 을 이용하여 실수, 백분율, 평균과 표준편차, 평균평점과 표준편차, 최솟값, 최댓값, t-test, ANOVA, Scheffé test, Pearson’s correlation coefficients 및 Stepwise multiple regression

Self-care performance of chronic hemodialysis patients has been considered to be an important factor in increasing the quality of life by effectively managing chronic diseases. The patients’ self-care performance has been confirmed to be related to sleep disorders as a physical factor, and depression, which is a psychological factor, as well as the resilience to the stress that arises from dialysis, are related as well. Therefore, this study aims to provide basic data for the development of a nursing intervention program to improve self-care performance by identifying the factors affecting the self-care performance of chronic hemodialysis patients.

This study is based on a descriptive research design, and 84 patients undergoing hemodialysis in a hemodialysis unit of an upper-level general hospital in B Metropolitan City were selected as participants. Data collection occurred from August 17, 2020, to September 5, 2020, through a structured self-report survey by obtaining approvals from the Institutional Review Board (IRB) of K University and the Nursing Department of K University Hospital located in B Metropolitan City.

For measuring the self-care performance, a tool developed by Song Mi-ryung (1999) for hemodialysis patients that was modified and supplemented by Cho Mi-kyung and Choi Myung-Ae (2007) was used, and for measuring sleep disorder, a tool developed by Oh Jin-ju, Song Mi-soon, and Kim Shin-mi (1998) that was modified and supplemented by this study’s investigator was used. For depression, BDI-I, a version of the Beck Depression Inventory (1967) standardized by Han Hongmu et al. (1986) that is centered around the standard Korean population, was used, and for resilience, a self-reporting resilience measuring tool developed by Kim Eun-young (2019) for hemodialysis patients was used.

Data analysis was performed using the IBM SPSS Win 25.0 program, and real number, percentage, mean ± standard deviation, mean grade ± standard deviation, minimums and maximums, t-test, ANOVA, Scheffé test, Pearson’s correlation coefficients, and Stepwise multiple regression were all utilized.
